This position is for a Home Health Physical Therapist to provide skilled care to patients in East Portland, OR and surrounding areas. The role involves a standard 8-hour day shift schedule with a 1:1 patient ratio, up to 6 visits daily. Successful candidates will possess a minimum of one year of home health experience and a current BCLS certification. While flexibility is valued, the facility may cancel one shift every two weeks.

Responsibilities:
- Provide skilled physical therapy services to patients in their homes.
- Develop and implement individualized treatment plans.
- Document patient progress and treatment in EPIC Remote Client and Rover app.
- Maintain accurate and timely charting.
- Manage a caseload of up to 6 patients per day.
- Adhere to all company policies and procedures.
- Travel to patients' homes within a designated radius (~60 miles max, occasionally).
- Maintain professional appearance (business casual dress code).
Required Skills & Certifications:
- Minimum 1 year of skilled home health experience.
- Current BCLS certification.
- Valid driver's license and reliable personal transportation.
Preferred Skills & Certifications:
- Experience with EPIC Remote Client and Rover app.
Special Considerations:
- Facility may cancel one shift per pay period (every two weeks).
- Occasional maximum driving radius of approximately 60 miles (60-90 minutes total driving time per day).
- RTO restrictions apply; confirmation required with hiring manager.
